Pathways Pregnancy Care Coordination Program

What is Pathways?
Pathways is a pregnancy care coordination service.  Pregnancy can be an exciting yet confusing time for many women.  Our experienced Care Coordinators can help make your life easier.  No matter where you get your prenatal care, you can sign up with a Pathways Pregnancy Care Coordinator.

Your Pregnancy Care Coordinator will:

  • Provide information on pregnancy and newborn care
  • Be someone to talk to during your pregnancy, in stressful times or whenever you need a listening ear
  • Help you achieve your future goals
  • Help you locate services, such as

    Medical care

  • Make referrals for:

    Prenatal care
    Education classes

Participation is VOLUNTARY
Refusing to participate in the program will not affect your health benefits in any way
